This heater terminal nut has locking washer, to ensure that it stays well tightened and maintains a good electrical contact between the electrical cable and the heater terminal.
Use a 3/8" spanner to tighten this nut, and a 1/4" spanner to hold the heater terminal just underneath. It is essential that you immobilse the heater terminal while you are tightening the nut, because it is fairly fragile and you risk shearing it off the element otherwise.
